Sex-hormone-bindingglobulin(SHBG)isabeta-globulinwhichspecificallybindssteroidhormones;itisinvolvedinthetransportofsexsteroidsinplasma.ThemainsiteofSHBGsynthesisisassumedtobethehepatocytes.TheproductionofSHBGisregulatedbyandrogen/estrogenbalance,thyroidhormones,anddietaryfactors,amongothers.TheconcentrationofSHBGisakeyfactorregulatingtheirdistributionbetweenprotein-boundandfreestates.SHBGconcentrationdeterminationisprimarilysignificantintheevaluationofmilddisordersofandrogenmetabolismanditallowsdetectionofwomenwithhirsutismwhoarelikelytoreacttoestrogentherapy.Testosterone/SHBG-ratioscorrelatewellwithbothmeasuredandcalculatedvaluesforfreetestosteronethusaidtodistinguishbetweensubjectswithexcessiveandrogenactivityandnormalindividuals.SHBGgenepolymorphismsarelinkedwithpolycysticovarysyndromeandtype2diabetesmellitus.
